Job Summary:
Clinical Pharmacist is responsible for different functions through Collaborative Drug Therapy Management (CDTM) for ensuring the safe, appropriate and cost-effective use of medicines and provides patient care. Under direction of Pharmacy management will assist to plan, to organize and to monitor for delivery of clinical pharmacy services at Indus hospital & health Network.

Roles and Responsibilities:
- To provide a Clinical Pharmacy Service to entire Medical and Surgical Specialties & to participate with consultant rounds
- To work in collaboration with healthcare professionals for enhancing patient safety, to ensure safe and effective management practices.
- To review patient profile on daily basis, identify prescription error, to resolve relevant issues before dispensing, to identify prescribing or administration errors, assessing potential risk & strategies to manage this risk. Making clinical interventions in prescribing or administration errors, and document.
- Physical patient monitoring in specific units/specialty wise for assessment to collect specification, interview, patient to identify their good of therapy (write detail notes patient reconciliation in HMIS)
- The aim to improve compliance, to coordinate with multidisciplinary team in educating patients, attendants, doctors, nurses and other paramedical staff about prescribed pharmaceuticals for Inpatient, OPD patients as well patient who visit emergency.
- To achieve medicines reconciliation processes on admission level to enhances patient safety
- In coordination with the primacy physician valuate patient-specific pharmacotherapy by evaluating appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
- Designs a comprehensive pharmacotherapy plan for patient-specific problems and persuasively justifies recommendations based on patient-specific pharmacologic, pharmacokinetic, pharmacodynamics, pharmacogenomics, Pharmaco economic, ethical, legal and evidence-based information or standard practice guidelines.
- Clinical Pharmacist is responsible to enter detail note on admission in progress note and patient profile. routine notes when review patient profiles.
