Education and Experience:
- Must be a Registered Pharmacist or hold a PharmD degree.
- Prior experience in telehealth, clinical pharmacy, or medication management is an advantage.
- Strong communication skills and ability to coordinate with multidisciplinary teams.
- Experience with Charm EHR and GoTo Connect is highly preferred.
- Detail-oriented, organized, and able to work efficiently in a fast-paced environment.
- Remote-work ready: stable internet, quiet work setup, and ability to work U.S. business hours.
Roles and Responsibilities:
Responsibilities:
- Review prescriptions, refill requests, and medication orders for accuracy and safety.
- Provide patient counseling, medication guidance, and address therapy-related questions.
- Collaborate with providers and care teams to ensure safe and effective medication use.
- Support clinical operations including prior authorizations, medication reviews, and documentation.
- Maintain complete and accurate records in Charm EHR.
- Uphold HIPAA standards and ensure strict patient confidentiality.
